Applications

Applications are reviewed on a first-come, first-served basis. An application packet consists of:

(1) Completed Membership Application (No blank spaces please)
(1) Completed Applicant Essay
(3) Completed Membership Recommendations
Parents’ last year's Federal tax return (1040) or Social Services statement
A photocopy of applicant’s Social Security card

Print out the Membership Application, Applicant Essay, and three copies of the Membership Recommendations forms which are available through the links above. Please carefully follow the instructions that are included on each form. Don't forget to include a copy of parents' last year's tax return (1040) or Social Services statement and a photocopy of applicant’s Social Security card. Complete application packets can be submitted at our office or mailed to:
