Change of Major Form
Complete Section I of this form whenever you need to change or add majors within the College of Arts and Sciences. Turn in the completed form to the front desk in 221 Varner Hall. It is recommended that you meet with an Academic Adviser to discuss your new degree program.
Senior/Graduation Check Form
Complete this form when you believe you are within one year of completing your degree requirements (91+ credits) to obtain an unofficial evaluation of your remaining course requirements. This will help you to schedule your final year of classes and avoid missing any of the requirements to complete your degree.
Note: this is not your Application for Degree. The Undergraduate Application for Degree must be completed through the Oakland University Academic Records Office website before the established deadline for the semester in which you intend to complete all degree requirements. The Undergraduate Application for Degree is your formal request to the university to award your degree.
Undergraduate Request to Participate in Commencement Ceremony
Use this form if you wish to request to walk earlier than your planned Oakland University commencement ceremony. Please read the form carefully to see if your planned graduation date makes you eligible to complete this request.
Unless you have an approved request form, Winter and Spring semester graduates can participate in the May commencement ceremony, while Summer and Fall semester graduates can participate in the December commencement ceremony.
Change of Minor/Concentration Form (CAS)
Use this form to declare or change a minor or concentration within the College of Arts and Sciences. It is recommended that you meet with an adviser or the coordinator for the minor or concentration you wish to declare before completing this form.
Concentration/Minor Authorization Form (non-CAS)
Use this form to declare a minor or concentration in an area outside of the College of Arts and Sciences (ex: a business minor, etc.). You should meet with the adviser or coordinator for the minor or concentration you wish to declare in order to complete this form together.
Override Request Form
Use this form to request a registration override if you need to register for a course for which you are completing the necessary prerequisite at another college or university.
